Authorization

REPAIR AUTHORIZATION AGREEMENT

If an insurance carrier is paying for all or part of my repair cost, I authorize and direct payment to Impressive Auto Body and Paint LLC and its affiliated companies. I assign all insurance proceeds for this claim to the Shop up to the full invoice amount. This assignment is irrevocable until paid in full. I authorize the Shop to negotiate all Supplemental Repairs directly with my insurance carrier on my behalf.

I, the undersigned, hereby authorize Impressive Auto Body and Paint LLC to perform the repairs described in the estimate provided. I understand and agree to the following:

1. The repair estimate is based on visible damage only. Additional damage may be discovered during disassembly, which will require supplemental authorization.
2. I authorize Impressive Auto Body and Paint LLC to operate the vehicle for testing, inspection, and quality control purposes.
3. Payment is due upon completion of repairs unless prior arrangements have been made with my insurance company.
4. Florida Mechanic's Lien Notice: Pursuant to Florida Statutes Section 713.585, the repair facility has the right to retain possession of the vehicle until all charges for labor and parts are paid in full. If charges are not paid within 30 days after written notice is sent by certified mail, the vehicle may be sold at public auction to satisfy the lien.
5. Storage charges of $45.00 per day will apply for vehicles left more than 3 days after notification of completion.
6. I certify that I am the legal owner or authorized representative of the vehicle described herein.
